On September 3, 2007 at 2106 eastern daylight time, a Cessna 172C, N1301Y, collided with the water in Lake Erie approximately one-quarter mile off the east end of Kelley's Island, Ohio. The airplane had just departed on runway 09 (2,203 feet by 50 feet) at the Kelley's Island Municipal Airport (89D). The private pilot and one passenger were fatally injured. A second passenger received serious injuries. The 14 CFR Part 91 personal flight was operating in visual meteorological conditions without a flight plan. The flight originated just prior to the accident with an intended destination of Lima, Ohio.
